Stacey Jirak died after the minivan she was riding in was struck by a sport-utility vehicle on County Rd. 2 near Juniper Street, said Lt. Carl Bock, Scott County spokesman. Around 4 p.m.

Friday, the westbound SUV drifted into the eastbound lane and forced a car into a ditch northeast of New Prague, authorities said. No one was hurt in the car. The minivan, also traveling east, tried to veer off the road but was hit on the side by the SUV. Jirak and two other minivan passengers were hurt. The others, a 15- year-old and an 8-year-old who are related to Jirak, were taken to a hospital in New Prague and released later Friday.

Jirak died Friday evening. Everyone involved in the accident was wearing a seat belt, no charges have been filed and the investigation continues, Bock said. Kelli D. He was admitted Aug. 9 to the Methodist Hospital Wound Clinic, where, the complaint said, blood tests showed he was malnourished.

He was discharged from the hospital about two weeks later to another nursing home and died about five weeks after that. C.K. had required a soft diet and thickened liquids because he had missing teeth and a swallowing problem. On a series of visits, his daughter found him with inadequate meals such as a piece of corned beef on top of a hard roll and soup that needed to be thickened, the complaint said.
